Extract from the State Register of Population, including electronic
Extract (attestation) from the register - is a reproduction of a part of the State Register of Population or other information resources of the Public Services Agency, in paper form (issued on a standardized blank) or in the form of an electronic document (electronic signature is required for submission of the application), PDF type, which will be placed on the Government Citizen's Portal (MCabinet).
The extract containing data from the State Register of Population or other information resources of the Public Services Agency (attestation) shall be issued on a standardized blank at the request of natural persons, regarding:
- the registration at domicile and/or temporary residence or lack thereof ;
- the issue of identity documents or lack thereof ;
- the citizenship of the Republic of Moldova ;
- the renunciation or withdrawal of citizenship of the Republic of Moldova ;
- the assignment of the state identification number of the natural person (IDNP) ;
- the issue of identity documents with previous personal data;
- the non-belonging to the citizenship of the Republic of Moldova;
- the acquisition of citizenship of the Republic of Moldova;
- the persons registered at domicile and/or temporary residence at a concrete address;
- the issue of an immigration authorization;
- the composition of the family;
- the repatriation of citizens of the Republic of Moldova;
- other information contained in the State Register of Population.
The application for the issuance of attestations shall be submitted in person or by the authorized representative by a power of attorney authenticated by a notary or other persons authorized by law to perform such function or by the authorized representative on the basis of electronic signature and recorded in the Automated Information System "Register of powers of representation based on electronic signature" (MPower).
Submission of the application by other persons is allowed in the following special situations:
- in the case of a child, including a child left without parental care or a child temporarily left without parental care over whom guardianship or curatorship is established, the child's legal representative (one of the parents, guardian, curator) acts as the applicant. The concept of child does not include a minor who has acquired full capacity of exercise until the age of 18 years under Article 26 paragraphs (2) and (3) of the Civil Code of the Republic of Moldova;
- in the case of a child whose parents are minors or whose only parent is a minor, the legal representative of the minor parents/only parent who is a minor shall act as the applicant;
- in the case of a child separated from his or her parents, including a child placed in a family-type placement service, with the exception of guardianship/curatorship, or in a residential-type placement service, the application shall be submitted by the territorial guardianship authority within whose territorial jurisdiction the child's place of stay or placement is located, with the mandatory presence of the child;
- in the case of a person subject to a legal protection measure in the form of guardianship, the legal representative (guardian) shall act as the applicant, with the mandatory presence of the person represented;
- in the case of an immobilized person, a person under coerced treatment, a person under preventive arrest or in detention, the application for the issuance of the certificate may be submitted by the representative authorized by a power of attorney authenticated by a notary or other persons authorized by law to do so or by a power of representation based on electronic signature, recorded in the Register of powers of representation based on electronic signature.

The application for the issuance of attestations shall be submitted:
1) online by accessing the Apply button, for the issuance of the extract concerning: the registration at domicile and/or temporary residence or lack thereof; the issuance of identity documents or lack thereof; the belonging to the citizenship of the Republic of Moldova; the loss of citizenship of the Republic of Moldova; the assignment of the state identification number of the natural person (IDNP). Electronic signature is required for the submission of the application (the issued extracts will be released exclusively in electronic form on the MCabinet Citizen's Portal).
2) at any identity document issuing service (SIID), including within the Multifunctional Centre of the Public Services Agency;
3) by ordering the service on the telephone numbers of the Customer Support Service (14 909 or (022) 25 70 70), for the issuance of the extract concerning: the issuance of identity documents or lack thereof; the belonging to the citizenship of the Republic of Moldova; the registration at domicile/temporary residence or lack thereof; the authorization for emigration.
4) at the Unified Public Service Delivery Centers (CUPS) within the Local Public Authorities (LPA), for the issuance of the extract concerning: the issuance of identity documents or lack thereof; the registration at domicile and/or temporary residence or lack thereof; persons registered at domicile and/or temporary residence at a concrete address; the issuance of identity documents with previous personal data; the renunciation or withdrawal of the citizenship of the Republic of Moldova; the authorization for emigration; the assignment of the state identification number of the natural person (IDNP); the repatriation of citizens of the Republic of Moldova.

- Identifying the person, verifying the documents submitted for the issuance of the attestation.
- Drawing up the order, entering the order information and saving the form data.

- Drawing up the text of the attestation and its registration.
- 4Step 4. Issuing the extract
Handing over the extract (on paper) containing the information from the State Register of Population or other informational resources of the PSA at the counter of the PSA subdivision or the result of the service will be an Extract in the form of an electronic document (PDF type), which will be placed on the Government Citizen's Portal (MCabinet).
The issued extract will be automatically generated, will contain the required information from the State Register of Population, a QR code and will be signed with the electronic signature of the head of the Public Services Agency.
