Notice of annulment and/or modification of the civil status act
The notice on the annulment and/or modification of the civil status act is the civil status document confirming the fact of some additions and/or rectifications in the civil status acts of its holder, a citizen of the Republic of Moldova.
The notice on the annulment and/or amendment of the civil status act reflects the fact of modifications of the person's name, surname, date and/or place of birth, nationality and other data that have been changed in the content of the civil status act on the basis of which the civil status documents were issued.
The grounds for making modifications to the civil status act are:
- CSD decision on the approval of the modifications in the respective civil status act;
- the request of the holder of the civil status act or his/her legal representative for the rectification of the name and/or surname on the basis of the spelling rules of the Romanian language;
- previous civil status act;
- judgment of the court establishing the inaccuracies in the civil status acts.
The notice on the annulment and/or modification of the civil status act shall be submitted to the authority concerned together with the civil status certificate or extract to which the notice relates and shall be valid throughout the period of validity of the certificate or extract concerned.
The notice on the annulment and/or modification of the civil status act shall be issued to:
- the holder of the civil status document;
- first- to second-degree relatives (presenting civil status certificates proving the degree of kinship);
- the legal representative of a person whose capacity is restricted or in respect of whom a court order has been issued for legal guardianship (guardian or curator);
- the representative authorized by a power of attorney, authenticated by a notary or by a power of attorney granted through the automated information system "Register of powers of attorney based on electronic signature" (MPower).

- 1Step 1. Submitting the application
The application shall be submitted in the following ways:
1) online - by clicking the Apply button. The online service is accessible for people with an electronic or mobile signature;
2) at the Civil Status Department's Directorate for Hearing and Issuing Civil Status Documents (Chisinau mun., 11/1 M.Viteazul Street, 2nd floor);
3) at the Civil Status Service in the applicant's place of residence, including within the Multifunctional Centre of the Public Services Agency;
4) at the Diplomatic Missions or Consular Offices of the Republic of Moldova accredited abroad (DMCO);
5) at the Unified Public Service Delivery Centers (CUPS) within the Local Public Authorities.
When submitting the application, the MDelivery delivery service can also be requested, which ensures the delivery of the civil status document directly to the applicant's place of residence, both in the Republic of Moldova and abroad.

Issuing the civil status act following the operation of modification in the civil status act.
- 4Step 4. Picking up the notice
The notice of annulment and/or modification of the civil status ACT shall be delivered, against signature, either personally to the holder, or to his/her legal representative, or to a person having a notarized power of attorney or a power of attorney based on electronic signature, recorded in the Register of Powers of Attorney based on electronic signature (MPower), which entitles him/her to receive the document:
1) at the counter of DAEDSC of the CSD, at the Information and Issuing Office of the Multifunctional Centre of the Public Services Agency;
2) abroad, at the DMCO of the Republic of Moldova;
3) delivery to the requested address through the government delivery service MDelivery.
