Extract from the integrated information system of the Border Police

The following documents shall be attached to the application submitted by the natural person, as appropriate: Copy of the identity card when requesting the release of extracts containing official information with limited accessibility concerning the applicant herself; the copy of the identity card of one of the parents, the copy of the birth certificate of the minor child, in the event that the parent/s, acting as their legal representative, requests the release of official information with limited accessibility concerning the minor(s); if the request for the release of information is submitted by the representative of the rights holder/lawyer, it is completed and signed by the representative/lawyer with the attachment of copies of the identity cards of the representative/lawyer card and the representative, the copy of the attorney's power of attorney/mandate, authenticated in the appropriate manner and certifying the right to request and receive information from the General Inspectorate of the Border Police; the copy of the identity card, the copy of the vehicle registration certificate or another document attesting the right of possession or use of the vehicle, in case of requesting information about the means of transport.

At the request of the legal entity, the following documents are attached, as the case may be: Copy of the registration certificate of the legal entity, copy of the vehicle registration certificate or other document certifying the right of possession or use of the vehicle; the authenticated copy of the power of attorney issued by the legal entity in the case of submitting the application through a representative, the copy of the legal entity's registration certificate, the copy of the vehicle's registration certificate or another document certifying the right of possession or use of the vehicle.
