Form on the contribution period completed on the territory of the Republic of Moldova
The form on the confirmation of the period of contribution completed on the territory of the Republic of Moldova is issued at the request of the competent institution for persons who have legally worked on the territory of the Republic of Moldova. The form shall be sent to the competent institution in the territory of one of the Contracting States to the Agreement in the field of social security to which the Republic of Moldova is a party for the purpose of aggregating insurance periods. The insured person who has been legally employed in the territory of one of the Contracting States party to the Agreement in the field of social security to which the Republic of Moldova is a party, or the representative designated by the proxy.

- 1Step 1. Submission of the application and necessary documents
The application is submitted:
1. On the territory of the Republic of Moldova:
- at any TOSI;
- at the central office of NOSI.
2. Outside the Republic of Moldova - at the competent institution of the Contracting State party to the Agreement in the field of social security.
- 2Step 2. Issue the form negotiated between the parties and send it to the competent institution/authority in the territory of the other Contracting State
The form shall be sent to the competent institution of the Contracting State party to the Agreement in the social security field.
